Supporting documentation can be uploaded now, or can be mailed. Applications will not be considered complete until the NDUS receives ALL required information, which includes application pages 1 and 2, and the certification letter from school district superintendent or school principal.

Please fill out this application fully and truthfully.


Teaching Shortage Areas

Please check the 2016/17 teaching shortage area below in which you have been employed full-time for the entire 2016-17 academic year. You must hold a full-time employment contract at a qualifying North Dakota school, in a designated teaching shortage area, to qualify.

This is not a retroactive award. You must be employed in a qualifying 2016-17 teaching shortage area in the 2016-17 academic year to qualify. The exception to this requirement is for applicants who received funding from this program in in the most recent award year (2015-16.) These applicants may continue to be funded under the area that is no longer considered a “shortage” if the applicant is still employed fulltime in the same 2015-16 shortage area (i.e. Applicant was funded in 2015-16 under “Art” may still qualify in 2016-17 if teaching in “Art”, even though Art is not a shortage area in 2016-17. All application requirements must be met.)

Signature and Certification

In order to certify and complete your application after submission, you must submit the letter of certification signed by your school superintendent or school principal. The form can be found by clicking here.

Applications will not be considered complete until the NDUS receives the completed application and signed letter of certification.

Submit the signed letter of certification to the NDUS financial aid office by uploading it below. As an alternative, the signed letter of certification can be faxed to 701-328-2979 or it can be mailed or delivered to:

Letter of Certification Upload

Note: Your application will not be completed until you have submitted your signed letter of certification. Please complete the application process by May 3, 2017. You can return to the online form and upload the signed letter of certification later or you can follow the instructions above to fax or mail the letter of certification to the NDUS office.

Submit Application

  • I hereby certify that the information contained on this application is true and correct.
  • I understand that I have the responsibility to apply for the Teacher Shortage Loan Forgiveness Program each year during the application period.
  • I understand that my application will not be considered if incomplete. Complete applications include the 2017 application and the signed letter of certification from my school superintendent or principal.
  • I give my employer permission to release to the North Dakota University System (NDUS) any information that may be needed to verify my application. I give permission to the financial institution listed on this application to release to the NDUS financial and loan information, and for the NDUS to share with the financial institution and the Bank of North Dakota, financial and loan information necessary to verify eligibility and process payment.
  • I further understand that any Teacher Shortage Loan Forgiveness payments made to my lender on my behalf must be applied only to outstanding balances on qualifying, non-defaulted loans. Any excess payment received by me from my lender, must be immediately returned to the NDUS.
  • I understand that applications will be approved in priority order and that funding is contingent upon continued legislative appropriations.
  • I acknowledge that any payment received under the Teacher Shortage Loan Forgiveness Program may be considered taxable income for income tax purposes in the year it is written off.
